If you are under 65 years of age and receive funding support for prosthetics you will be eligible for the NDIS and should apply for the Scheme once it becomes available in your area.

To check your eligibility, please contact the National Disability Insurance Agency (NDIA) directly on ph: 1800 800 110 and ask for an NDIS Access Request Form to be sent to you.

In general, to be eligible for NDIS funding you must:

What happens if I am accepted as an NDIS participant?

If you are accepted as an NDIS participant, responsibility for funding your items will move from the state-based scheme to the NDIS. HPOS can continue to assist until your NDIS Plan is in place.

If you are accepted as an NDIS participant, please let us know so we can update our records and assist with your NDIS planning.

HPOS clients can email hpos@health.nsw.gov.au or call us on 02 4921 4151.

Can I still use HPOS?

HPOS is a registered provider to the National Disability Insurance Agency (NDIA) and can continue to provide prosthetic and orthotic supports approved in your NDIS Plan. We simply need to be included as a provider in your NDIS Plan, along with a budget for the required services.

What happens if I am NOT eligible for the NDIS?

If you currently receive funding assistance but do not meet the NDIS criteria, nothing will change.

What are the benefits of using HPOS as an NDIS provider?

If you already use HPOS, nominating us as a provider means minimal disruption to the process you are already used to. The only difference is that the funding for the services will come from the NDIS.

When you start your planning meetings, we recommend you contact us to ensure any consumable items and a repairs budget for your existing items are included in your NDIS Plan.

Can I use HPOS if I decide to self-manage my NDIS Plan?

Yes. If you decide to self-manage you can request your items directly from HPOS.
